ASH Disclosure Protocol
ITEM #01:
The Disclosure Policy for Conflict of Interest Issues within the
Society is addressed in the following three (3) documents:
- “American Society of Hypertension,
Inc. (ASH) - Reportable Relationships and Other Disclosures
Policy for Educational Activities” --
this document reviews the disclosure protocol for persons involved
in educational activities of the Society (Click

- “American Society of Hypertension, Inc. (ASH) - Conflict
of Interest Policy for Responsible Persons: Officers, Board Members,
Committee Members, Executive Staff & Legal Counsel” – this
document reviews the disclosure protocol for responsible persons
of the Society (Click
here)
- “American Society of Hypertension,
Inc. (ASH) - Guidelines for Relationships with For-Profit
Corporations” – this
document reviews the guidelines for relationships between the
Society and industry (Click
here)
ITEM #02:
Current Members of the Society may obtain copies of completed
Disclosure Forms by Responsible Person(s)*, providing that
the following criteria are met: (a) a representation in writing
is made that any such requests are in good faith and not made
for a purpose which is in the interest of a business or object
other than the business or object of the Society, or for any
purpose to harm the Society or the person(s) whose disclosure(s)
are requested, and (b) submission of a signed confidentiality
statement (click here).
To obtain completed Disclosure Forms for ASH Responsible
Persons current ASH Members must forward a written request to the
ASH Advisory Body on Conflict of Interest c/o the ASH Office, stating
the names of the Responsible Person(s)* for whom Disclosure Forms
are requested, enclosing a completed Certification Form (Click
